¶ … Hospital Health Administration Innovation for elder care

Several research problems covering the response of acute care hospital administration to the ever increasing population of the elderly like decision making, development of in-patient units, need for higher education levels, etc. I have referenced 10 pieces of research that cover elder care as well as innovation. Below is a list of those references. I will briefly summarize the articles then give the rationale for selecting each article.

Omachonu, V., & Einspruch, N. (2010). Innovation in Healthcare Delivery Systems: A Conceptual Framework. The Innovation Journal: The Public Sector Innovation Journal, 15(1).

This research article defines healthcare innovation and explores the place of innovation in healthcare. It then develops a conceptual framework which outlines the factors promoting healthcare innovation. The elements give researchers room to answer many questions as far as healthcare innovation is concerned. The article concludes by giving ten research questions which are key in the healthcare innovation field. The argument is that in answering these questions, we can curve a route to advance healthcare innovation in the future. The questions presented include:

What effects will innovation have on healthcare organizations' major stakeholders?

What implications will specific healthcare innovations have on diagnosis, treatment, education, prevention, outreach and research?

With the limited research in the healthcare innovation field, I have to admit that it is interesting seeing research being carried out in this field. The research will help other researchers get answers to their questions and will open their eyes to what is possible so as to advance the already carried research and simplify it for easy comprehension by patients and makers of policy.

The thesis gives a detailed take into the varied techniques of organization of teams, applicability and theory of an agile model of development of software, trends in leadership and mechanisms that promote innovation showing that if implemented properly small teams have the capability to produce products that can compete globally. This thesis first covers the already accepted models of development of software and then covers deeply the modern perspectives in software development. It then explores the place of agile models in self-organization as well as the use of creative management and leadership models in the process of development. Further, it covers the innovation methodologies that a small team would make use of to identify those products that can make a difference. It explores the models that are already accepted in the process of transformation of an idea into a product. The gist of the thesis is the learning curve required and the appreciation of that learning curve and the results that will accrue due to such appreciation.

This article was selected to prove that people are indeed the drivers of healthcare innovation. People can make use of current principles in the development of software, team organization, leadership and innovation to pool resources efficiently to complete projects that can result in less expensive projects that can give a challenge to the corporate giants.

This study explores the different preferences stakeholders have for innovations, and also quantifies the priorities of stakeholders as far as costs and benefits are concerned. What to adopt therefore varies from stakeholder to stakeholder as their view on costs and benefits vary. Analytic hierarchy, a technique used to support decisions, was made use of in quantifying the stakeholder preferences in nine IT healthcare innovations. The innovations were selected based on expert judgments and literature review. The decision making criteria grounded on costs and benefits were clearly defined. The selected criteria involved efficiency improvements, satisfaction with the care process, required investment and gains in health. The stakeholders weighted the decision criteria and then prioritized them and then selected the best innovations based on their expectations of the innovation's performance on the decision criteria. The researchers found out that different stakeholders (nurses, patients, managers, policy makers and insurers) arrived to different conclusions based on their preferences. The differences could be explained by the expected benefits to each stakeholder rather than the costs to be incurred by each stakeholder. The study was the first to explore quantitatively the position of different stakeholders regarding healthcare innovation. It also gave a formula for studying the different preferences of stakeholders.

What rationale was used in selecting this study? Given that stakeholders hold different opinions and that can impede implementation of various innovations, it is crucial to come up with a way to reach a compromise so that processes move faster and everyone is made happy. While the study brought to light the possible differences that might surface, it does not provide any solutions to these problems. What would this mean for key decision makers?

4) Osborne, S.P., Radnor, Z., & Nasi, G. (2013). A New Theory for Public Service Management? Toward a (Public) Service-Dominant Approach. American Review of Public Administration, 43(2), 135-158 10.1177/0275074012466935

The article makes an argument that public management theory has never been fit for purpose. It is argued that it is also flawed in two key ways - it has a focus on inter-organizational processes when the reality of the time is that delivery of public services is inter-organizational, and that it is influenced by management theory drawn from the world of manufacturing that does not take into account the world of service delivery. This article then makes an argument for 'public service domination' approach. This is more relevant to present public management than it is applicable to manufacturing which was the main body of its focus. It was thus argued that the approach brings forth an innovative and forward thinking contribution to public management theory in an era of New Public Governance. The conclusion was made by exploring what implications this approach might have in four areas of public management. A research agenda is also set for a public-service dominant theory applicable in the future.

This article gives a possible solution to the problems in organizations that can happen in the areas of elder care hospital innovation. Public management is a key facet to that. Effectively handling public management will lead to innovation. This is the reason for my choosing of this article.

5) Xing, K., Ness, D., & Lin, F. (2013). A service innovation model for synergistic community transformation: Integrated application of systems theory and product-service systems. Journal of Cleaner Production, 43, 93-102.

This piece of research gave a conceptual model to be used in testing relational learning moderating effect on the association between innovation and knowledge strategies. To achieve this, the study was done in some health centers. Both tacit and explicit knowledge are widely accepted to play a fundamental role in organizational innovation. Nonetheless, even with the presence of much research on the link between how an effective an innovation process is and knowledge management, questions still arise on this link. The article covered this topic again but with data derived from a portion of hospitals in Spain covering relational learning, KM and innovation outcomes. The research evidences that the availability of wide knowledge results in much better outcomes in innovation. Plus, the research found out that health institutions that had a hand in relational learning had a better chance at promoting innovations.

The 21st century has presented professionals with a need to innovate. This was rationale behind the choosing of this paper. The presence of a good knowledge base will result in a vibrant and successful process of innovation. More knowledge presents us with more options on what can be pursued. Health institutions should invest the time in building knowledge so as to promote innovations.

This article explored Nurses Improving Care of Health system Elders (NICHE) and the way it avails resources to hospitals giving them resources and tools to help in the implementation of initiatives aimed at improving health outcomes for the elderly and the families of the elderly. Starting 2011, the members have been involved in evaluation process of the programs, which are designed to examine the progress made internally towards the development, sustenance and dissemination of NICHE. The article gives a description of self-evaluation of NICHE sites and gives a report of the initial data of self-evaluation in 180 hospitals in North America. Members of NICHE use the following criteria to evaluate the various facets of geriatric acute care program: organizational structures, competence of the staff, interdisciplinary process and resources, care of the environment, quality metrics, approaches that are patient-centered and family-centered and leadership.
