Maag (2006) researched social skills training (SST) in a public school setting, and concluded that therapists frequently lack the expertise, resources, personnel or inclination to effectively use the method (p. 14). The overall ineffectiveness, according to Maag (2006) may be attributed to methodological flaws, including the tendency to over-generalize, and resistance to the institution of long-standing recommendations for enhancing the effectiveness of SST therapy. The factors that appear to inhibit the effectiveness of an SST approach are not present in a clinical setting. Further research is needed to explore the influence of SST on outcomes for patients with CD who are also engaged in CBT.

Study Purpose

The purpose of this study is to explore the effectiveness of combined individual interventions for adolescents with Conduct Disorder (CD). The research shows that Cognitive Behavioral Therapy (CBT) used in individual and family therapy, social skills training (SST), and pharmacological treatment with Fluoxetine have all been effective in reducing the symptoms and behaviors associated with Conduct Disorder. All subjects were assessed and treated for substance abuse problems regardless of group assignment. Follow along interventions for substance abuse will continue throughout the duration of the study. The objective of the study is to determine whether combined social skills and cognitive intervention approaches are more effective than pharmacotherapy alone, or more effective when paired with pharmacotherapy.

Research Questions & Hypothesis

The literature provides evidence of the effectiveness of various therapeutic approaches to treating conduct disorder (CD) in adolescents; however, the research has tended to focus on single therapeutic intervention types rather than combined therapeutic interventions. To further the research on effective therapeutic interventions for adolescent males with CD, the following research questions are proposed.

Does the use of CBT in family therapy provide an effective approach to intervention for adolescent males with CD? To what degree are unacceptable behaviors of adolescent males with CD mediated by the use of CBT in family therapy? Do adolescent males with CD perceive that the use of CBT in family therapy effectively alters their disordered thinking of in the short-term?

Does SST in combination with CBT provide an effective approach to intervention for adolescent males with CD? To what degree are unacceptable behaviors mediated by SST in combination with CBT in family therapy? Do adolescent males with CD perceive that the use of SST in combination with CBT in family therapy effectively alters their disordered thinking in the short-term?

Are there measurable differences in the level or frequency of expression of unacceptable behaviors, and perceptions of self-esteem and self-control in adolescents with CD across the three treatment groups?

Does pharmacotherapy provide an effective intervention for adolescent males with CD? To what degree are unacceptable behaviors mediated by pharmacotherapy? Do adolescent males with CD perceive that pharmacotherapy is effective in reducing their unacceptable behaviors?

Hypothesis: Combined social and cognitive interventions in family therapy are more effective without pharmacological intervention in the treatment of CD in adolescent males.

Participants

Subjects were adolescent males previously diagnosed as having conduct disorder (CD) and new to the family therapy milieu. The subjects were randomly divided into two experimental groups and one control group. The treatment and control groups were as follows: (A) CBT in family therapy plus Social Skills Training (SST) plus a placebo (B) Administration of Fluoxetine; (C) CBT in family therapy plus Social Skills Training (SST) (Control Group). A total of 9 subjects were included in the study. All treatment took place in clinical settings and was configured to be individual or family therapy rather than peer-group treatment.

Instrumentation

The unit of analysis is the behavioral and cognitive processing performance changes in individual subjects (patients). Changes in the expression of problem behavior are noted by clinicians. Self-perception scores of the changes in cognitive processing were recorded on the surveys and two CBT instruments. The level of measurement is ordinal as dictated by the scales used in the formal CBT tools, and on the Likert scale used for the structured surveys. The Cognitive Therapy Awareness Scale (CTAS) and the Cognitive Behavior Therapy Supervision Checklist (CBTSC) will be used to measure the effectiveness of the treatment groups (Sudak, et al., 2001;… [END OF PREVIEW]
